This report was commissioned to identify persistent issues that affect emergency responder interoperability as they are reported in a selection of recent inquiries, reviews, and other pertinent
materials, available from unclassified and publically accessible sources.

The identification and analysis of these themes establishes a historical and contextual evidence base, to assist the Joint Emergency Services Interoperability Programme (JESIP).
